Privacy Policy
This is a comprehensive, high-level Privacy Policy and Service Agreement for Alliance Physiotherapy. It is drafted with a clinical and legal tone to ensure it looks professional for a high-end home-based service.
Privacy Policy and Clinical Terms of Service
Alliance Physiotherapy (the “Practice”) is committed to protecting the privacy, dignity, and personal information of our patients. As a provider of specialized home-based physiotherapy, we adhere to strict professional ethics and data protection standards. This document outlines our comprehensive protocols regarding data collection, patient confidentiality, and service delivery.
1. Scope of Data Collection
To deliver safe and effective rehabilitative care, Alliance Physiotherapy collects and processes the following categories of information:
-
Personal Identification: Legal name, date of birth, gender, and verified contact information.
-
Medical & Clinical History: Comprehensive health records, including pre-existing conditions, surgical history, current medications, allergies, and diagnostic imaging (e.g., MRI, CT scans, X-rays).
-
Biometric & Assessment Data: Baseline physical measurements, range of motion (ROM) scores, muscle strength grading, and functional mobility assessments conducted during the initial evaluation.
-
Session Documentation: Detailed clinical notes recorded after every home visit, capturing the patient’s response to specific therapeutic interventions.
2. Utilization of Information
All data collected is utilized solely for the following professional purposes:
-
Clinical Customization: Developing a rigorous, evidence-based treatment plan tailored to the patient’s specific pathology.
-
Risk Management: Ensuring that the prescribed physical exercises and manual therapies do not contraindicate any existing medical conditions.
-
Operational Logistics: Managing the complex scheduling required for our 9:00 AM to 6:00 PM mobile service window.
-
Communication: Providing treatment updates, progress reports, and essential appointment reminders.
3. Professional Service Standards (The 2-Hour Protocol)
Alliance Physiotherapy operates on a premium service model. Unlike traditional 30-minute clinic visits, we provide a dedicated 2-hour (120-minute) appointment block.
-
Dedicated Focus: This extended duration is mandatory to facilitate deep-tissue mobilization, neurological re-education, and supervised corrective exercises without the constraints of a rushed environment.
-
Home Environment Safety: A portion of this time is dedicated to assessing the patient’s home environment to mitigate fall risks and optimize ergonomics for daily living.
4. Confidentiality and Data Integrity
We maintain a “Zero Leakage” policy regarding patient data.
-
Internal Access: Access to patient records is strictly limited to the primary therapist and authorized administrative staff under a signed non-disclosure agreement (NDA).
-
Third-Party Disclosure: Alliance Physiotherapy will never sell, rent, or trade your personal or medical data to third-party marketers. Information is only shared with other medical professionals (e.g., your referring surgeon) upon your explicit written consent.
-
Digital Security: All digital records are stored using industry-standard encryption and secure server protocols to protect against unauthorized access or cyber threats.
5. Patient Consent and Home-Based Protocols
By engaging our services, the patient acknowledges:
-
Informed Consent: The patient understands the nature of the physical therapy interventions and provides consent for the therapist to perform manual techniques within their home.
-
Accurate Disclosure: The patient agrees to provide a full and honest medical history. Alliance Physiotherapy is not liable for complications arising from the non-disclosure of medical facts.
-
Environment: The patient agrees to provide a safe and appropriate space within the home for the 2-hour treatment session.
6. Rights of the Data Subject
Patients of Alliance Physiotherapy hold the following rights:
-
Right to Access: You may request a summary of your clinical progress and copies of your treatment records at any time.
-
Right to Rectification: You have the right to correct any inaccuracies in your personal or medical files.

Right to Erasure: Upon the conclusion of the treatment cycle, you may request the permanent deletion of your records, subject to statutory medical record-keeping requirements.
